Please fill out the form below and select the assessment session you plan to attend. Please keep in mind that you MUST attend one assessment session in order to qualify for consideration for the cohort. If you cannot attend, please send a representative with sufficient organizational knowledge in your place. This can include a board member or a staff person who has the institutional knowledge to complete the assessment. 

Please be punctual and prepared to remain on the call for the full two-hour session.

AGAIN, ANY ORGANIZATION THAT DOES NOT PARTICIPATE IN THE ASSESSMENT WILL BE INELIGIBLE FOR CONSIDERATION.
